The individual licensees and facilities listed agreed to enter into an informal agreement (Consent Decree) with the Board for multiple violations or their license to practice was suspended or revoked for violation of W. Va. Code § 30-27 and/or the Board’s Legislative Rules. In Board-offered Consent Decrees, which is in lieu of a hearing and is a legally binding agreement, the Board requires the licensee to meet certain requests as a form of discipline for unlawful practices. The agreements consist of the salon/shop owner agreeing to be assessed for administrative fines and fees and inspections and sanitation/infection control training by a Board Inspector for 4 weeks, if warranted. It should be noted that the names of facilities and individuals listed may be similar to the names of parties who have not had disciplinary action taken against them.
